Kids’ Brazilian Jiu Jitsu Classes
Purpose:
This Brazilian Ji Jitsu Program (“BJJ”) in the Frisco masjid aims to train the community in self-defense. In martial arts, Brazilian jiu-jitsu is the most effective in that regard. It teaches participants how to escape difficult positions and protect oneself. While self-defense has benefits, there is a much more significant benefit: confidence. BJJ is a great way to gain physical competence and feel more confident.
5 - 8 year old class - 3:30 PM to 4:30 PM
8 - 12 year old class - 4:45 PM to 6:00 PM
Requirements for Students:
We will need the following from the students:
- Athletic clothing
- T-shirt
- Athletic joggers (Islamic attire is required, no shorts)
- Bring your own water
- Make sure kids arrive on time
- Make sure kids have used restroom prior to attending
- Parents must sign-in
- Parents are encouraged to stay in the hall to observe, while kids are practicing
- One-time signed waiver form
- We will take breaks during the salah times
Equipment:
The kids only need to wear athletic clothing and don’t need a GI. We will be teaching them No Gi jiu jitsu, which only requires a t-shirt and pants. This will make the class easily accessible.
Waiver of Liability - please read carefully
ISLAMIC CENTER OF FRISCO, and I, , as a parent, guardian or authorized representative of , as a minor, or the adult signing Student/Participant below, in consideration of the terms and conditions herein and in further consideration of gaining access to participating in Jiu Jitsu or being able to participate, spectate or in any other way using or accessing ISLAMIC CENTER OF FRISCO building and gym facilities, Jiu Jitsu classes, programs, matches, exercises, instruction, instructors, equipment, exercise, competitions, workouts, methods, philosophies, or in any other manner hereby agree to and affirm, any and all terms and conditions of this ASSUMPTION OF RISK, RELEASE, WAIVER OF LIABILITY AND INDEMNITY AGREEMENT (“Agreement”) as a condition precedent to and condition of gaining access to ISLAMIC CENTER OF FRISCO, or use of, or participating in any Jiu Jitsu activities at any and all times, on or off the premises of ISLAMIC CENTER OF FRISCO. This agreement supersedes any prior oral or written agreements and no other agreements or understandings exist between the Parties other than as provided herein.
1. Representation and Warranties of Participant
The authorized signer represents and warrants to ISLAMIC CENTER OF FRISCO, that participant has no known or undisclosed medical, physical, mental or any other health conditions or any pre-existing medical history or conditions that exist or create any type of concern, special concerns, or undue risks or concerns of any kind of character. To the extent of any known or unknown health risks, health conditions of any kind, mental or physical in nature Participant fully assumes any and all such risks, health risks, if any, disclosed or undisclosed of any type, kind of character and hereby completely and fully waives and releases and indemnifies ISLAMIC CENTER OF FRISCO, and all Releases from any and all claims, demands, causes of action, damages and liability arising from or relating thereto any activities Participant participates in or through ISLAMIC CENTER OF FRISCO, of any kind or description, on or off ISLAMIC CENTER OF FRISCO premises.
2. The adult signatory below and/or parents, legal guardian or authorized representative of any minor Participant will instruct the minor participant that prior to participating in Brazilian Jiu-Jitsu at ISLAMIC CENTER OF FRISCO, he or she should inspect the facilities and equipment to be used, and shall any adult, and if he/she believes anything is unsafe, the Participant shall immediately advise the officials and ISLAMIC CENTER OF FRISCO of such conditions and REFUSE TO PARTICIPATE. The undersigned agree and understand that, if at any time, anything is felt unsafe, such person shall REFUSE TO PARTICIPATE and avoid any area or equipment thought to be unsafe and refuse to participate in any activities, matches, competitions, classes or any exercises thought to be unsafe and to avoid any unsafe areas or events.
3. I, the undersigned signer, and Participant understands and acknowledges:
a.) There are inherent risks, known risks and unknown risks and dangers associated with participation in Brazilian Jiu-Jitsu at ISLAMIC CENTER OF FRISCO, or sponsored by or though ISLAMIC CENTER OF FRISCO, which could result in bodily injury, impairment, partial or total disability, paralysis and/or death on or off the premises.
c.)The economic loss and/or damages which could result from any risks, inherent risks, unknown risks and dangers including but not limited to the above could be severe. With knowledge and disclosure, any and all such damages and claims are waived and released hereby.
d.)There may be additional risks and dangers not known to ISLAMIC CENTER OF FRISCO, or Participant. By signing this Agreement and participating, spectating or otherwise, Participant fully assumes and indemnifies and hold harmless ISLAMIC CENTER OF FRISCO, and releases from any and all known and unknown risks of every type or kind and any and all related or unrelated injuries, damages and costs of every kind and description.
Signer/Participant consents to and approves of ISLAMIC CENTER OF FRISCO, and its agents providing aid, and care to any injured Participant or Signer with knowledge that such aid or administered by ISLAMIC CENTER OF FRISCO, or its agents to Participant/Signer could impact or potentially worsen or effect the condition of injured Participant/Signer. Participant/Signer consent to ISLAMIC CENTER OF FRISCO, and its agents rendering aid and care, which may include, but not be limited to calling 911 and taking any other actions under the emergency circumstance calling for aid and care.
Participant/Signer hereby assumes any and all risks of every kind and character, known or unknown, including the rendering of any care or aid to any injured Participant and completely and fully, now and forever, release, indemnify, hold harmless and waive any claims, actions, damages or causes of action of any kind and arising from or relating to participating in, the teaching of, practice, exercising, matches, competition, engaging in any activities at ISLAMIC CENTER OF FRISCO and its facilities, events or any sponsored activities of any kind or character in any way related, or not to Jiu Jitsu as to and including ISLAMIC CENTER OF FRISCO, and its agents and Releasee from any and all activities and including the taking of any efforts to aid injured Participant or Signer and indemnify and hold harmless ISLAMIC CENTER OF FRISCO, and Releasee therefrom; including any ambulance, and any other transportation costs and medical/health care bills of any kind or description. Participant/Signer and any other transportation costs and medical/health care bills of any kind or description. Participant/Signer agree to assign, transfer and pay any insurance proceeds, insurance coverage for the benefit of ISLAMIC CENTER OF FRISCO to the extent of any loss or claims asserted against ISLAMIC CENTER OF FRISCO.
Participant/Signer hereby assume all such risks and financial responsibility, loss and damages of every kind and description arising from or relating to any activities at ISLAMIC CENTER OF FRISCO, and indemnifies ISLAMIC CENTER OF FRISCO, and Releasee therefrom.
I, the undersigned and Participant in activities and instruction at ISLAMIC CENTER OF FRISCO, after this NOTICE AND DISCLOSURE OF RISKS, KNOWN AND UNKNOWN, INHERENT OR OTHERWISE, ELECT TO PARTICIPATE IN BRAZILIAN JIU-JITSU AT ISLAMIC CENTER OF FRISCO, OR OFF THE PREMISES WITH KNOWLEDGE OF THE ABOVE as initialed by me and agreeing to the terms and conditions of this Agreement.
4. The undersigned/Participant fully ASSUMES any such RISKS and the sole responsibility for any loss and/or damages of every kind and description arising from or relating to or following anu injury, injuries, disability, harm, paralysis and/or death, physical or mental impairment, however caused, and whether caused, in whole or part, by any type of negligence of Releasees including, but not limited to ISLAMIC CENTER OF FRISCO, and its owners and agents, while at or on the premises at ISLAMIC CENTER OF FRISCO, or off the premises and/or participating in any activities sponsored by, promoted by ISLAMIC CENTER OF FRISCO, now and forever, regardless of location.
I/We HEREBY RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE ISLAMIC CENTER OF FRISCO, including its owners, instructors, members, Releasees, Licensor, managers, promoters, lessors and lessees of premises used to conduct the martial arts event or program, premises and event inspectors, insurers, underwriters, consultants and others who give recommendations, directions, or instructions to engage in risk evaluation or loss control activities regarding the martial arts facility or events held at any such facility and each of them. Their directors, officers, agents, employees, all for the purpose herein referred to as “Releasee or Releasees” including ISLAMIC CENTER OF FRISCO, from any and all liability to the undersigned, my/our personal representatives, assigns, executors, heirs and next of kin for any and all claims, actions, demands, losses and damages of every kind and description therefore on account of any injuries, including but not limited to the death of the participant or damage to property, on account of any injuries, including but not limited to the death of the participant or damage to the property, arising out of or relating to the event(s) caused alleged to be caused in whole or in part by the negligence of any type or kind or other conduct of the Releasee or otherwise.
5. I/WE HEREBY acknowledge that THE ACTIVITIES OF THE EVENT(S) ARE VERY DANGEROUS and involve the risk of serious injury and/or death and/or property damage. Each of THE UNDERSIGNED also expressly acknowledges that INJURIES RECEIVED MAY BE COMPUNDED OR INCREASED BY NEGLIGENT RESCUE OPERATIONS OR PROCEDURES OF THE RELEASEES OF THEIR AGENTS.
6. EACH OF THE UNDERSIGNED further expressly agrees that any and all of the above and foregoing releases, hold harmless, waivers, and indemnities of this agreement shall apply to all activities, events, on or off the premises and is intended to be as broad and inclusive as is permitted by the law of the Province or State in which the event is conducted and apply to any and all activities and that if any portion is held invalid, it is assumptions of risk, waivers, indemnity and hold harmless provisions are cumulative in nature and shall be so construed and not limited to any singular type of conduct, event or occurrence. This intent of this Agreement is to be broad and provide ISLAMIC CENTER OF FRISCO, and Releasees full and complete releases and indemnifications for any and all injuries, damages, claims or causes of action of any type or kind.
7. On behalf of the participant and individually, the undersigned Individual(s) and/or legal guardian(s) for the minor participants executes this Waiver and Release. If, despite the release, the participant makes a claim against any of the Releasees, the parent(s) and/or legal guardian(s) will reimburse the Releasee for any money which they have paid to the participant, or on his behalf, and hold them harmless.
I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW AND AS PROVIDED ABOVE IN THIS AGREEMENT.
